The proposition
What was claimed
The FDA takes an approval action on Savara's Biologics License Application for molgramostim inhalation solution (proposed trade name MOLBREEVI) for the treatment of autoimmune pulmonary alveolar proteinosis, with an approval action date on or before 2026-11-22, the PDUFA target action date disclosed in Savara's Form 10-Q filed 2026-08-11.

- Target rungA step on the 12-rung ladder: rungs 1 to 4 are the evidence moving, rungs 5 to 7 are a guideline body, regulator, or payer acting. The ladder, rung by rung
- Rung 6: Regulatory action — Label/indication change, approval action, REMS modification, or withdrawal by FDA, EMA, PMDA, Health Canada, etc.

The estimate
The number and the baselines it has to beat
The alternatives it was registered against, each with the reasoning that produced it.
ESTIMATE FROM THE SHAPE OF THE CLASS, NOT AN ENUMERATED COUNT. Original BLAs under Priority Review with a positive randomised phase 3 trial are approved at their first goal date in a large majority of cases. A Refusal to File does not, to the drafter's understanding, start a review cycle, so this is read as a first cycle. The calendar's own met-goal-date share for disclosed PDUFA dates does not exist yet (docs/foundations/short-horizon-reference-classes.md §4.1).
A clock-only model: the goal date is inside the window and FDA meets most goal dates, discounted because the date has already been extended once by a major amendment.
An analyst reading the same public record on 2026-09-24 — a positive phase 3 trial published in the New England Journal of Medicine, Breakthrough designation and Priority Review, no approved alternative, a sponsor building a launch — would call approval by the goal date likely, with the drug-substance transfer, the nebulizer and whatever the April major amendment answered as the main risks. Estimated by reconstructing that read, not by surveying analysts.
Regulatory action (FDA): offered on judgment, with no calibration figure until 15 in the class have resolved.

Resolution
The rule that settles the forecast
Resolves YES if the openFDA Drugs@FDA record returned by https://api.fda.gov/drug/drugsfda.json?search=openfda.generic_name:%22molgramostim%22 (or, if that returns nothing, by https://api.fda.gov/drug/drugsfda.json?search=products.active_ingredients.name:%22molgramostim%22) contains an application whose ORIG submission has submission_status AP and a submission_status_date on or before 2026-11-22, and the Drugs@FDA approval letter or label for that application names autoimmune pulmonary alveolar proteinosis in its indication. A proper name that adds a suffix to molgramostim still matches. An approval for a narrower autoimmune pulmonary alveolar proteinosis population than the applicant sought still resolves YES. Resolves NO if no approval action is dated on or before 2026-11-22: a complete response letter, a further goal-date extension, withdrawal or no action. The goal date is frozen as disclosed at the cutoff; a later extension does not move it, and an approval dated after 2026-11-22 resolves NO as occurred_after_deadline. 2026-11-22 is a Sunday; an approval action dated on or before it counts whatever the weekday. Read no earlier than 2026-12-06. If openFDA has not refreshed by then, the Drugs@FDA web record (https://www.accessdata.fda.gov/scripts/cder/daf/, searched by the name molgramostim) governs; if Drugs@FDA does not show the application either, FDA's Purple Book (https://purplebooksearch.fda.gov/) record of a licensed molgramostim product with Savara or a Savara subsidiary as licensee governs, read with the approval letter for the indication. The application number was not public at the cutoff, so the rule reads by generic name; if the product carries a proprietary name, MOLBREEVI or another, the generic name still identifies it. Savara expected CDER to hold primary jurisdiction (SRC-002); if CBER licenses the product instead, the Purple Book record and the CBER approval letter govern on the same terms. Resolves unresolvable only if none of these sources can be read after the read floor. Resolves VOID, with the reason recorded, only if FDA withdraws every public record of approval actions named above without an equivalent public successor, so that no approval date for this product can be read; while any of them publishes it, the rule stands.

What this record will show once it resolves
Beside every result, the count for its class (FDA action by goal date): registered, resolved, unresolved, and expired. One forecast is one instance, not a rate.
- The finding
- As the rule above reads it: occurred on time, did not occur, or left unscored.
- The readers and their sources
- Two reader roles read the source named above: Resolver A, clinical methodology; Resolver B, payer evidence. Who adjudicated it is named here, with the sources they read when the adjudication lists them. Being named as a reader means applying the frozen rule to the public record; it is not support for the estimate, the method, or NextConsensus.
- The number against its baselines
- 75% beside three baselines, each scored by the same rule (squared error), so a reader can see how the record did against each of them.
Reasoning
What the number rests on
The facts it was built from are below, as they stood at the cutoff, with every source archived; each baseline above carries the reasoning that produced it. The written mechanism and the conditions that would make the proposition ill-posed were frozen with the record and are held in the ledger, which the hash on this page covers.

SRC-001Savara 10-Q for the quarter ended 2026-06-30, filed 2026-08-11. States that MOLBREEVI is the proposed trade name for molgramostim inhalation solution, an inhaled GM-CSF and Savara's sole program, not approved in any indication; that in February 2026 the FDA formally filed the BLA for MOLBREEVI in autoimmune pulmonary alveolar proteinosis (autoimmune PAP) and granted Priority Review; and that in April 2026 the FDA determined Savara's responses to information requests constituted a major amendment, extending the PDUFA target action date by three months to November 22, 2026. Also records Fast Track, Breakthrough Therapy and Orphan Drug designations, an EMA marketing authorisation application validated in March 2026, and a royalty agreement paying Savara on FDA approval on or before 2027-03-31. -
SRC-002Savara 10-K for the year ended 2025-12-31, filed 2026-03-13. States Savara believes MOLBREEVI, a drug-device combination with PARI's eFlow nebulizer, will be regulated as a drug under the primary jurisdiction of FDA's Center for Drug Evaluation and Research (CDER), with CDRH reviewing the nebulizer. Records the BLA history: submitted March 2025; a Refusal to File letter in May 2025, which Savara says did not request or recommend additional efficacy studies; a Type A meeting aligning with FDA on drug substance comparability between GEMA and Fujifilm; resubmission in December 2025 with Fujifilm as the drug substance supplier; and filing with Priority Review in February 2026. Records IMPALA-2, a randomized, double-blind, placebo-controlled phase 3 trial that enrolled 164 patients and met its primary endpoint (change from baseline in DLCO at Week 24), with statistically significant secondary results, published in the New England Journal of Medicine in August 2025; that no medicines are approved for autoimmune PAP in the U.S.; and that certain manufacturing and supply vendors had not yet received an FDA inspection.
