Medical affairs
- If the decision is earlier
- Check which materials apply the reversed position to a population the trial did not study.
- If the decision is later
- Keep the narrower position visible instead of withdrawing the whole class.
Part 4 of 4 Following a decision through the record
A safety signal arrives, and the first question is what position it actually overturns. What reverses is a recommendation for a named population, in a named document. Freeze that population before the outcome is known, and the record will tell you what reversed and what survived.
For medical affairs and labeling teams deciding whether a safety signal reaches their population.
01 The position
Menopausal hormone therapy was recommended for the prevention of chronic disease in postmenopausal women. That is a position: it has a population, an indication and a source. 'Hormone therapy was recommended' is not a position, and it cannot be tested.
The estrogen-plus-progestin trial that halted in 2002 enrolled women with a mean age of 63. It tested chronic disease prevention, not treatment of vasomotor symptoms. Those two populations separate the position that reversed from the one that survived.
| Date | Body | Act | Population | State |
|---|---|---|---|---|
| WHI data safety board | Trial halted Trial halt notice | Postmenopausal women, mean age 63, estrogen plus progestin | Open | |
| JAMA | Trial results published Trial paper | Postmenopausal women, mean age 63, estrogen plus progestin | Open | |
| FDA | Labeling changed Label | Menopausal hormone therapy for chronic disease prevention | Open | |
| USPSTF | Recommendation against routine use Recommendation statement | Postmenopausal women, chronic disease prevention | Open | |
| NAMS | Position statement Position statement | Vasomotor symptoms, women under 60 or within 10 years of menopause | Open |
The 2005 recommendation addressed chronic disease prevention. The 2017 statement addressed vasomotor symptoms in early menopause. They are different positions sharing a drug.
02 What held
The position that reversed was chronic disease prevention. The position that did not reverse was treatment of vasomotor symptoms in early menopause. The recommendation against chronic disease prevention was applied to the second population anyway, and the 2017 statement corrected the over-application.
Two positions sharing a drug. Only one of them reversed.
Our reading Register a reversal against a frozen population, not against a drug. The hormone case shows why the distinction matters; it does not establish how often it does.
03 The test
Copy the recommendation's population verbatim at the cutoff. Then read the later document and ask whether it still recommends the technology for that population. The test is whether the recommendation is there, not whether the evidence improved or the tone softened.
Our reading The 2017 statement corrects an over-application; it is not a second reversal. Calling it one loses the population that never changed.
What each team changes if the decision comes earlier or later than planned.
Dates, acts and populations come from the named document; a study's population is the one it enrolled. A body's account of its own rule is a record of the account, consensus is a dated summary rather than a source, and "Our reading" marks interpretation, not a finding.
Which decision matters to your team, and by what date? Include the population and the plan it would inform.
